About The Position

Roze Room is seeking a Per Diem Weekend Triage Registered Nurse to join their team. This is an in-office position located at their Reseda facility, with working hours on Saturdays and Sundays from 8 am to 6 pm. The role involves providing skilled and palliative care assessments, instructions, and coordinating services with the hospice team during after-hours, weekends, and holidays. The triage nurse will manage incoming calls from patients, families, and other stakeholders, with the flexibility to work from home at least two nights per week. Collaboration with physicians and the multidisciplinary hospice team is essential for updating the plan of care, implementing prescribed medical treatments (including pain and symptom management), and offering education and support to patients and caregivers.

Requirements

Graduate of an accredited school of nursing.
Current RN License within the State.
Minimum of one-year experience as a professional nurse within the last 3 years.
Must be able to utilize computers and be comfortable with electronic medical records.

Nice To Haves

Hospice/Home Health Triage Experience preferred.
Certification in Hospice and Palliative Nursing desired and encouraged.
Hospice experience preferred.

Responsibilities

Provide skilled and palliative care assessments and instructions.
Coordinate services with the hospice team after hours, weekends, and holidays.

Work cooperatively with physicians and the hospice team of multi-disciplinary professionals to update the plan of care.
