About The Position

Concordia Visiting Nurses is seeking a dedicated and adaptable Triage Registered Nurse to join their team. This role is crucial in coordinating and overseeing patient care within a supportive and collaborative environment, aiming to make a meaningful impact on patient outcomes and community well-being. This is a part-time/casual position working 32 hours per month, offering flexibility in hours to balance patient needs and personal schedules.

Requirements

  • Current Registered Nurse (RN) license in the state of Pennsylvania
  • Minimum of 2 years of current experience as a professional nurse in a medical/surgical environment
  • Minimum of 1 year home health experience recommended
  • Possesses strong assessment, problem solving and decision-making skills
  • Expertise in pain management, symptom control, crisis intervention, and family support
  • Skilled in critical thinking, communication, negotiation, collaboration and customer service
  • Excellent organizational skills with attention to detail
  • Outstanding communication skills (both written and verbal); must communicate in a warm, professional manner

Responsibilities

  • Utilize company telephone and laptop computer to access patient records during after-hours.
  • Consult the on-call schedule to identify on-call clinicians and their assignments.
  • Assess patient signs and symptoms via telecommunication, reporting reactions to treatments, including drugs, and changes in patient's physical or emotional condition to the physician.
  • Triage patients via telecommunication, assessing indicated further care by CVN staff or providing necessary care over the phone.
  • Delegate visit responsibility to on-call clinicians or notify patients to seek emergent care via hospital emergency room when indicated.
  • Instruct and counsel patients and caregivers regarding patient care requirements in the home.
  • Enter notes into the Case Communication note as an On-Call Note to document communication between the Triage Nurse and the patient, including instructions given and the clinician's name and time directive was given.
  • Complete the intake process for after-hour referral calls.
