This is a part-time (30 or 32 hours per week) remote position for a Triage Registered Nurse (RN) within the Patient Access and Care Team. The role involves utilizing the nursing process and established protocols to assess patient needs telephonically, determine the appropriate level of care, provide education, establish a plan of care, and communicate follow-up instructions. The nurse will escalate to on-call providers when necessary, prioritize patient interactions by acuity, and apply evidence-based practice. Responsibilities also include implementing strategies to reduce patient risk, assessing patient and family readiness to learn, supporting shared governance, participating in quality improvement initiatives, and engaging in professional development. The role requires effective communication, conflict resolution, and collaboration with healthcare team members. Promoting a culture of safety, ethical decision-making, and patient-centered care are key aspects. The nurse must demonstrate knowledge of age-appropriate care and principles of growth and development. Accurate maintenance of electronic medical records, communication of patient status, and participation in team meetings and special projects are also required.
