The Triage Registered Nurse (RN) - Patient Access and Care Team (PACT) role is a part-time position requiring 32 hours per week. This remote role involves using the nursing process to assess patient needs telephonically, determine the appropriate level of care, provide education, establish a plan of care, and communicate follow-up instructions. The RN will prioritize patient interactions by acuity, apply evidence-based practice, implement strategies to reduce patient risk and increase safety, and support shared governance and quality improvement initiatives. The role also involves participating in professional development, demonstrating effective communication and conflict resolution, promoting a culture of safety, and applying ethical decision-making. The RN must be knowledgeable about age-specific patient care, accurately maintain clinical records in the Electronic Medical Record, and communicate patient status to the interdisciplinary team. Responsibilities also include participating in team meetings, developing health information resources, and assisting with interpreting department policies.
