About The Position

Haven Hospice is seeking a passionate, dedicated Triage RN to join their team. This role involves providing calm, confident support during critical moments and acting as a lifeline for patients and families after hours. The position offers an opportunity to guide, comfort, and coordinate care from home while working with a dedicated team. As a Hospice Triage Registered Nurse, you will answer hospice calls during nights, weekends, holidays, and emergencies, help schedule nurse visits, handle emergency patient admissions, and dispatch on-call staff. You will also talk with patients and caregivers to assess needs and provide support over the phone, communicate with doctors about patient updates and get new care orders, give instructions and guidance based on each patient’s care plan, coordinate after-hours needs such as equipment, supplies, and medications, and work with facility staff to support their patients during off-hours. All calls, care updates, and decisions must be documented clearly and accurately, ensuring all care and procedures follow state and federal rules.

Requirements

• Graduate of an accredited nursing school with current licensure to practice in state of operation.
• 1 year hospice nursing experience
• Nursing Compact RN license.

Nice To Haves

• 3+ years hospice nursing experience
• Experience in hospice or similar setting preferred.
• Prior experience with triage, preferred.

Responsibilities

• Answer hospice calls during nights, weekends, holidays, and emergencies.
• Help schedule nurse visits, handle emergency patient admissions, and dispatch on-call staff.
