About The Position

The Retreat Nursing Home is seeking a dedicated and detail-oriented Treatment / Skin Integrity Nurse to join our clinical team. This full-time position plays an important role in maintaining resident skin integrity, preventing pressure injuries, coordinating wound care, and ensuring treatments are completed and documented accurately. The ideal candidate will have strong clinical assessment skills, excellent attention to detail, and a commitment to providing high-quality care to our residents.

Requirements

  • Current RN or LPN license in the State of Georgia in good standing.
  • Strong assessment, documentation, organizational, and communication skills.
  • Ability to work collaboratively with nursing staff and the interdisciplinary care team.

Nice To Haves

  • Previous long-term care, skilled nursing, wound care, or treatment nurse experience preferred.
  • Wound care certification is a plus but is not required.
  • Knowledge of pressure injury prevention, wound management, infection prevention, and long-term care documentation standards preferred.

Responsibilities

  • Perform and document ordered wound care and other nursing treatments.
  • Conduct routine skin assessments and monitor residents for changes in skin condition.
  • Identify residents at risk for skin breakdown and assist with implementing appropriate preventative interventions.
  • Monitor pressure injuries, wounds, skin tears, surgical sites, and other skin conditions for healing or changes.
  • Measure, photograph, and document wounds in accordance with facility policy and applicable regulations.
  • Collaborate with the Director of Nursing, nursing staff, physicians, wound care providers, dietitian, therapy, and other members of the interdisciplinary team.
  • Ensure physician orders and treatment records accurately reflect current wound care and treatment plans.
  • Participate in wound rounds and communicate significant changes in resident condition promptly.
  • Assist with developing and updating care plans related to wounds, skin integrity, and preventative measures.
  • Monitor the use and effectiveness of pressure-relieving mattresses, cushions, positioning schedules, and other preventative interventions.
  • Review documentation to help ensure treatments and preventative measures are being completed as ordered.
  • Educate nursing assistants and licensed nursing staff regarding skin integrity, pressure injury prevention, positioning, and treatment protocols.
  • Assist with tracking and trending facility wound and skin integrity data for Quality Assurance and Performance Improvement (QAPI).
  • Maintain treatment supplies and notify appropriate staff of supply needs.
  • Assist nursing staff with resident care and other clinical responsibilities as needed.
