About The Position

This position is responsible for documenting resident conditions and nursing needs, implementing physician's orders, performing weekly skin audits, and managing wound care documentation. The Treatment LPN will assign nursing care, perform treatments, and participate in staff orientation and training. They will also assist physicians with tests and procedures, ensure adequate supplies, monitor safety regulations, and maintain clean and sanitary work and resident care areas. This role involves participating in care plan reviews, assisting with the Quality Assurance Program for wound care, communicating with resident families, and reporting wound status to the Medical Director and Director of Nursing. The position also oversees the Infection Control Program, prepares infection control reports, manages supplies economically, and reports resident observations. Coordination of nursing care, emergency measures, and promoting positive staff morale are key aspects. The LPN will answer resident call lights, ensure frequent resident checks, and report any suspected abuse immediately.

Requirements

Graduate of accredited LPN/LVN School of Nursing or RN School of Nursing
Valid RN license
Valid LPN license
Valid CPR certification
One year experience in nursing preferred.
Skilled in motivating and directing the workforce.
Ability to work cooperatively as a member of a team.
Ability to communicate effectively with the residents and their family members, and at all levels of the facility.
Ability to react decisively and quickly in emergency situations.
Knowledge of training techniques for clinical staff.
Ability to maintain confidentiality.

Nice To Haves

Wound care certification preferred.

Responsibilities

Documents the resident’s condition and nursing needs.
Accurately and promptly implements physicians’ orders.
Performs Weekly Skin Audits on all residents.
Provides weekly documentation on wound care log and nurses notes, to include measurements, treatments and response to treatment.
Assigns nursing care to team members in accordance with the resident’s needs and the person’s capabilities and qualifications.
Performs treatments for assigned resident’s and documents that treatment as required by Regency, state and federal regulations.
Participates in orientation and in-service training for personnel.
Assists physicians with special tests and procedures within the scope of state specified Nursing Practice Act.
Assures that adequate supplies are available to personnel who perform procedures that involve exposure to blood or body fluids.
Monitors Nursing service to ensure that they are following established safety regulations in the use of equipment and supplies.
Ensures that all nursing work areas (i.e. nurse’s station, med rooms etc.) are maintained in a clean and sanitary manner.
Ensures that all resident care rooms, treatment areas, etc., are maintained in a clean and safe manner.
Participates in the review and revision of care plans and assessments as necessary, but at least quarterly.
Assists with the development, implementation and maintenance of an ongoing Quality Assurance Program for the nursing department as pertaining to Wound Care Management.
Contacts families of residents with skin treatments on a weekly basis with an update.
Ensures Medical Director and Director of Nursing are made aware of wounds that are not responding to treatment weekly.
Consults with RD, Dietary Manager and PT on complicated wound healing.
Oversees the facility Infection Control Program, to insure each infection is monitored and follow-up labs as ordered.
Prepares the monthly infection control report with tracking and trends.
Ensures that supplies are utilized economically and that equipment is clean and maintained in a safe manner.
Reports and records pertinent observations and reactions regarding residents.
Coordinates nursing care of residents when scheduled for therapy or procedures by other departments.
Assists with or institutes emergency measures for sudden adverse developments in residents.
Promotes interpersonal relationships and develops staff morale.
Answers resident call lights and ensures that all residents are checked frequently.
Reports any suspected abuse to the Director of Nursing/Administrator immediately.
