About The Position

The Treatment LPN/RN is responsible for documenting resident conditions and nursing needs, accurately implementing physician's orders, and performing weekly skin audits. This role involves managing wound care, assigning nursing tasks, performing treatments, and participating in staff training. The position also requires assisting physicians, ensuring adequate supplies, maintaining clean and safe work and resident areas, and contributing to the Quality Assurance Program for wound care. Additionally, the Treatment LPN/RN will communicate with resident families, report on wound status to management, consult with other departments on complex cases, and oversee the facility's Infection Control Program. This includes monitoring infections, preparing monthly reports, and ensuring efficient use of supplies and equipment. The role also involves responding to resident needs, reporting suspected abuse, and promoting positive staff morale.

Requirements

Valid LPN or RN license.
Ability to document resident conditions and nursing needs.
Skill in accurately and promptly implementing physicians’ orders.
Proficiency in performing weekly skin audits.
Experience with wound care documentation, including measurements, treatments, and response to treatment.
Ability to assign nursing care effectively.
Competence in performing treatments and documenting them as required.
Willingness to participate in orientation and in-service training.
Ability to assist physicians with tests and procedures.
Knowledge of safety regulations for equipment and supplies.
Ability to maintain clean and sanitary work and resident areas.
Skill in reviewing and revising care plans and assessments.
Experience with Quality Assurance Programs, particularly in Wound Care Management.
Ability to communicate effectively with resident families.
Knowledge of infection control principles and practices.
Ability to monitor infections and follow up on lab orders.
Skill in preparing reports with tracking and trends.
Ability to utilize supplies economically and maintain equipment safely.
Competence in reporting and recording resident observations.
Ability to coordinate nursing care with other departments.
Skill in instituting emergency measures.
Ability to promote interpersonal relationships and staff morale.
Responsiveness to resident call lights and frequent resident checks.
Understanding of reporting procedures for suspected abuse.

Nice To Haves

Experience in a long-term care or skilled nursing facility.
Certification in wound care.
Familiarity with specific EMR/EHR systems used for documentation.

Responsibilities

Documents the resident’s condition and nursing needs.
Accurately and promptly implements physicians’ orders.
Performs weekly Skin Audits on all residents.
Provides weekly documentation on wound care log and nurses notes, including measurements, treatments, and response to treatment.
Assigns nursing care to team members based on resident needs and staff capabilities/qualifications.
Performs treatments for assigned residents and documents them according to regulations.
Participates in orientation and in-service training for personnel.
Assists physicians with special tests and procedures within the scope of the state's Nursing Practice Act.
Ensures adequate supplies are available for procedures involving blood or body fluid exposure.
Monitors nursing services to ensure adherence to safety regulations in equipment and supply usage.
Maintains a clean and sanitary environment in all nursing work areas (e.g., nurse’s station, med rooms).
Ensures resident care rooms and treatment areas are maintained in a clean and safe manner.
Participates in the review and revision of care plans and assessments at least quarterly.
Assists with the development, implementation, and maintenance of the nursing department's Quality Assurance Program, specifically for Wound Care Management.
Contacts resident families weekly to provide updates on skin treatments.
Ensures Medical Director & Director of Nursing are informed weekly about wounds not responding to treatment.
Consults with RD, Dietary Manager, and PT on complicated wound healing.
Oversees the facility's Infection Control Program, monitoring infections and following up on ordered labs.
Prepares the monthly infection control report, including tracking and trends.
Ensures infection control reports are completed for all infections.
Ensures supplies are utilized economically and equipment is clean and maintained safely.
Reports and records pertinent observations and reactions regarding residents.
Coordinates nursing care for residents scheduled for therapy or procedures.
Assists with or institutes emergency measures for sudden adverse resident developments.
Performs other assigned tasks.
Promotes interpersonal relationships and develops staff morale.
Answers resident call lights and ensures residents are checked frequently.
Reports any suspected abuse to the Director of Nursing/Administrator immediately.
