About The Position

Embrace Treatment Foster Care, an ncgCARE partner, is seeking a Foster Care Case Worker to join their team. The mission is to provide foster children with the best chance for a successful future through a CARE model offering support, training, and guidance to empower foster families. The Foster Care Case Worker plays a vital role in delivering coordinated, individualized care to the community by providing case management and support, child and family supervision, service coordination and continuity, and education and counseling.

Requirements

  • Master’s or Doctorate in Social Work or a related field (sociology, psychology, special education, or counseling) from an accredited institution with at least one year of experience in providing casework services to children and families.
  • Bachelor’s degree in Social Work or a related field with relevant experience in casework services or at least two years of experience in casework services with a bachelor’s degree in any field.
  • Must be authorized to work in the United States.

Responsibilities

  • Conduct interviews with children and families, and complete detailed home studies.
  • Develop individualized treatment and service plans that address each child’s unique needs.
  • Provide counseling and guidance to children and families throughout placement or discharge.
  • Oversee children in foster, resource, and adoptive homes, as well as independent living arrangements.
  • Perform regular assessments to determine the child’s needs for medical, psychosocial, educational, and nutritional services.
  • Link children to appropriate services, minimizing barriers to care and coordinating with other agencies to ensure comprehensive service access.
  • Assist in arranging referrals for medical, remedial, and dental services as outlined in each child’s treatment plan.
  • Monitor the ongoing progress of each case, evaluating the effectiveness of the care plan and collaborating with involved parties, including family assessment teams.
  • Build a supportive, trusting relationship with the child, promoting growth and well-being in alignment with the service plan.
  • Educate children and families on available resources and work closely with team members to prepare comprehensive case plans.
  • Conduct interviews, home studies, and assessments for children and foster families.
  • Develop and monitor individualized treatment and service plans for each child.
  • Coordinate medical, educational, counseling, and community support services.
  • Maintain regular communication with families, foster homes, and partner agencies to track progress and ensure quality care.
