The TFC community liaison will work under the supervision of the TFC Program Manager to oversee assigned program(s). Expected areas of focus include but are not limited to 1) program coordination, 2) staff / treatment foster care family coordination, 3) provision of direct services and participation in a multidisciplinary team approach, 4) cultural competency and diversity, 5) documentation, 6) productivity and efficiency, 7) staff /treat foster care family development and skill enhancement activities, 8) other duties and special projects as assigned and developed. Requirements

• Bachelor’s degree or higher in human services or health-related field (including, but not limited to counseling, social work, psychology, etc.)
• A minimum of two-year experience working in the social serves field and interacting/providing services for individuals and families.
• Valid drivers’ license and meets the driving privileges guidelines established in HR Policy 12.6 “Vehicle Safety and Client Transportation”
• Ability to demonstrate all functions of clinical program staff.
• Ability to manage time to meet all deadlines and prioritize projects.
• Ability to recognize and identify symptoms of mental illness and suicide and risk potential and respond appropriately.
• Good verbal, organizational, and written skills.

Nice To Haves

• Bilingual (English/Spanish) is preferred but not required.

Responsibilities

• Functions as primary coordinator for functions of the treatment foster care program under the supervision of the treatment foster care program manager as evidenced by: Completes all data tracking, collection, and management tasks related to program needs and contract requirements.
• Supports Program Manager in tasks to promote program adherence to contract, state, and federal requirements for program.
• Coordinates all aspects of new client program enrollment including intake, referral, coordination, and assignment coordination; coordinates all aspects of onboarding and retaining all new treatment foster families.
• Works collaboratively with program manager to identify and engage in growth opportunities.
• Coordinates and manages on-call schedules and forwarding.
• Coordinates and oversees any requirements and tasking of special contracts or projects to support program including data reporting, point-of-contact and outreach, and RFP or other written response/proposals.
• Acts as a bridge between functions of program manager, treatment coordinators, treatment foster families and clients to promote enhanced patient experience.
• Participates in community outreach and program promotion.
• Demonstrates excellent customer service to referring entities and community partners, attends trainings/meeting as assigned, and represents the organization in a positive and professional way in the community.
• Conducts all functions of TC position including ongoing services to TC caseload as determined by program need.
• Provides support for direct service staff under supervision of Program Manager as evidenced by: Coordinating and providing of initial training and orientation to new hires and to new treatment foster families in collaboration with Program Manager and state requirements.
• Supporting Program Manager with hiring manager tasks.
• Engaging in activities of ongoing training/support/and professional development with treatment foster families to meet program service quality and state requirements.
• Serving as the team liaison between other entities (including other departments, outside entities vendors).
• Identifying, developing, and facilitating in-house trainings in both group and individual format for treatment foster families.
• Supporting Program Manager to ensure all team members meet requirements related to staff training as identified by agency and regulatory standards.
• Participates as an active treatment team member serving children and their families with mental health and substance use issues in the TFC Program as evidenced by: Demonstrating the ability to recognize and identify symptoms of mental illness and substance use.
• Participating as a member of a multi-disciplinary team approach to treatment and recovery.
• Coordinating with both internal and external providers regarding service provision.
• Completing services delivery as needed by program (within appropriate scope of education/licensure).
• Enhance quality of service delivery including but not limited to outreach to treatment foster families, receipt and triage of treatment foster family’s concern/question, and crisis support.
• Providing other direct service functions as assigned including individual case assignments
• Provides services that are person-centered and culturally relevant.
• Providing a non-judgmental, open, and welcoming approach to diverse populations.
• Developing treatment foster care team members’ skills in offering services to diverse populations that are person-centered, culturally relevant, and non-judgmental.
• Documenting all services provided within expected time frames.
• Documenting all services provided in a manner that meets service definitions, regulatory standards, and agency expectations.
• Documenting all services provided in a manner that is person-centered and meets quality improvement goals.
• Supports Program Manager in overseeing program documentation to ensure that team members’ documentation meet requirements of agency, regularity entities, and quality improvement goals.
• Demonstrating excellent organizational and time management skills.
• Demonstrating the use of effective engagement skills.
• Consistently meeting established productivity standards and deadlines.
• Demonstrating the ability to support treatment foster families to ensure established productivity standards are met on a consistent basis.
• Participating in and completing all required trainings in an accurate and timely manner including foster care specific training of 40 hours.
• Identifying and requesting training or skill enhancement opportunities.
• Engaging in cross-training activities with other team members.
• Attending and participating in supervision sessions and demonstrating the ability to apply supervisory recommendations to work performance.
• Completes other administrative support duties and special projects as developed and assigned.
• Medical Records Maintenance, completes enrollment and maintenance of patient medical records and profiles in agency medical records.
