About The Position

Mindify Wellness and Care is seeking an outgoing, compassionate, and bilingual Medical Assistant & Patient Care Coordinator to help expand access to behavioral health services throughout New Jersey. This unique role combines clinical support, patient advocacy, community outreach, and care coordination. You'll work directly with older adults, caregivers, providers, and adult day care center staff to help individuals understand, access, and stay connected to behavioral health services. This position is ideal for someone who enjoys building relationships, traveling to different locations, educating communities, and making a meaningful impact on the lives of others.

Requirements

  • Medical Assistant Certification (CMA, CCMA, RMA, NCMA, or equivalent).
  • Fluent in both English and Spanish.
  • Excellent communication and interpersonal skills.
  • Strong public speaking and presentation abilities.
  • Comfortable working with older adults and diverse populations.
  • Strong organizational and time-management skills.
  • Proficiency with Microsoft Office and electronic medical record systems.
  • Valid driver's license and reliable transportation.
  • Willingness to travel throughout New Jersey.

Nice To Haves

  • Experience in behavioral health, psychiatry, primary care, community health, senior services, or adult day care settings.
  • Experience working with older adults.
  • Experience in patient advocacy, care coordination, or healthcare navigation.
  • Experience providing interpretation services in healthcare settings.

Responsibilities

  • Build trusting relationships with adult day care participants, caregivers, and staff.
  • Educate individuals about available psychiatric, therapy, and behavioral health services.
  • Conduct educational presentations and wellness discussions.
  • Help patients understand treatment options and available resources.
  • Identify individuals who may benefit from behavioral health services and facilitate referrals.
  • Reduce barriers to care by helping patients navigate available services.
  • Coordinate appointments and support continuity of care.
  • Follow up with patients regarding appointments and treatment participation.
  • Serve as a liaison between patients, families, providers, and adult day care center staff.
  • Assist patients in accessing appropriate behavioral health resources and services.
  • Support patient engagement and long-term treatment participation.
  • Support psychiatric and therapy providers during onsite clinic visits.
  • Assist with patient intake and appointment preparation.
  • Obtain and document vital signs when appropriate.
  • Coordinate patient flow during provider visits.
  • Assist with patient registration and required documentation.
  • Support providers with administrative and clinical duties within scope of practice.
  • Ensure patient records and clinical documentation are complete and accurate.
  • Provide Spanish-English interpretation for patients, families, and providers when needed.
  • Assist with communication during psychiatric and therapy appointments.
  • Help ensure patients receive culturally responsive and accessible care.
  • Develop strong working relationships with Administrators, Directors of Nursing, Social Workers, Activities Coordinators, and other community partners.
  • Promote collaboration that supports improved patient care and service access.
  • Identify opportunities to expand access to behavioral health services within assigned locations.
  • Represent Mindify Wellness and Care professionally at all times.
  • Maintain accurate documentation of patient interactions, referrals, outreach activities, and presentations.
  • Complete required reporting and documentation in a timely manner.
  • Maintain HIPAA compliance and patient confidentiality.
  • Communicate regularly with leadership regarding outreach activities, patient engagement, and referral opportunities.
