About The Position

The Long-Term Acute Care (LTAC) Registered Nurse provides specialized, ongoing care for medically complex adult patients. Operating within a hospital-affiliated LTAC unit in Columbia, MO, this role centers on continuous patient monitoring, complex clinical interventions, multi-system management, and collaborative discharge planning. You will work alongside an interdisciplinary care team—including physicians, therapists, and case managers—to observe subtle clinical trends, manage high-acuity needs, and optimize patient recovery.

Requirements

  • Associate Degree in Nursing (ADN) or Bachelor of Science in Nursing (BSN) from an accredited program.
  • Active, unencumbered Registered Nurse (RN) license in the state of Missouri (or a valid Multi-State/Compact RN License).
  • Basic Life Support (BLS) for Healthcare Providers (Required)
  • Demonstrable experience with complex inpatient acute or post-acute care, telemetry, wound management, and IV therapy.
  • Strong clinical judgment, interdisciplinary communication, and EHR documentation skills are essential.

Nice To Haves

  • Advanced Cardiovascular Life Support (ACLS) (Recommended)
  • Specialized LTAC, Progressive Care (PCCN), or Critical Care (CCRN) certification (Preferred)
  • Bachelor of Science in Nursing (BSN).
  • Hands-on experience with mechanical ventilation and tracheostomy care.
  • Proficiency with advanced care coordination platforms and hospital EHR systems.

Responsibilities

  • Perform detailed clinical assessments, monitor vital signs and lab trends, administer ordered medications, manage complex IV therapy/infusions, and provide specialized wound, dressing, and ostomy care.
  • Vigilantly monitor for acute clinical changes or complications; escalate findings promptly to attending physicians according to facility protocols.
  • Work closely with physicians, therapists, case managers, and family members to build, evaluate, and adjust individualized, comprehensive care plans.
  • Maintain precise, timely clinical documentation in the Electronic Health Record (EHR). Educate patients and caregivers on treatment plans, medication management, and post-discharge expectations.
