About The Position

This is a part-time position for a Travel Registered Nurse, RN, Case Manager. The role involves assessing, planning, evaluating, and coordinating patient care, including discharge planning and utilization needs. The Case Manager supports ministry and system goals for capacity management by achieving length of stay goals and identifying avoidable delays. They collaborate with the interprofessional team, establish partnerships with Patient Experience champions, and ensure seamless patient care transitions by identifying discharge needs and problem-solving continuity issues. The role also involves ensuring appropriate clinical information is provided to outside agencies, communicating effectively with the patient care team, and identifying/eliminating barriers to effective patient stay management. Professional development, maintaining licensure, and advocating for patient rights while balancing resources and hospital financial stewardship are key aspects. The Case Manager will refer quality issues and poor utilization of services to appropriate sources, coordinate/facilitate patient/family education, and educate hospital leadership and colleagues on current case management practices and trends. Consultation is provided to professional colleagues regarding patient stay options, financial considerations, psychosocial concerns, risk management, and ethical issues. Documentation of assessments, interventions, transitions, and outcomes is required in accordance with standards. The role requires utilization of computer systems and maintaining knowledge of applicable laws, regulations, and the Trinity Health Integrity and Compliance Program and Code of Conduct.

Requirements

  • Valid license as an RN.
  • BSN Required.
  • BLS Certification Required.
  • 3-5 years Case Management experience, hospital or community based.

  • Proficient in common software applications and electronic medical records. (Word, Excel, Power point).

Responsibilities

  • Assesses, plans, evaluates, and coordinates management of patient care including discharge planning and utilization needs.

  • Coordinates with the interprofessional team in meeting patient care goals.
  • Establishes partnerships with site Patient Experience champions to align efforts with patient satisfaction outcomes.
  • Identifies discharge needs, determines the appropriate level of care that the patient can transition to, coordinates planning, and problem solves continuity issues to ensure seamless continuum of patient care and prevent the patient from having to readmit to the hospital.
  • Ensures all appropriate clinical information is provided to outside agencies.
  • Communicates pertinent information to the appropriate members of the patient care team both effectively and professionally.
  • Identifies and eliminates barriers to effective patient stay management and promotes improvements in work processes within department, hospital, and community.
  • Pursues professional development opportunities and maintains licensure.
  • Advocates for patient legal, social, and ethical rights while balancing patient resources and hospital financial stewardship.
  • Refers quality issues and poor utilization of services to the appropriate sources.
  • Coordinates/facilitates patient/family education.
  • Educates hospital leadership and colleagues to current case management practices, patient coordination issues, and unit specific trends.
  • Provides consultation to professional colleagues and employees regarding patient stay options, financial considerations, psychosocial concerns, risk management, and ethical issues.
  • Documents pertinent assessments, interventions, transitions, and outcomes for patients in accordance with documentation standards, policies, procedures and/or guidelines.
  • Utilizes computer systems.
  • Maintains a working knowledge of applicable Federal, State, and local laws and regulations, Trinity Health Integrity and Compliance Program and Code of Conduct, as well as other policies and procedures to ensure adherence in a manner that reflects honest, ethical, and professional behavior.
