About The Position

This position is for a Travel Outreach RN Heart Failure Coordinator at Baylor University Medical Center in Dallas, Texas, within the Advanced Treatment and Transplant Clinic, focusing on heart failure patients. The role involves 90% travel throughout Texas to see patients, with occasional overnight stays. Requirements

  • Associate's degree level.
  • 2+ years of experience working with heart failure patients, or ICU in an inpatient setting.
  • Willingness to travel (overnight stay may be required at times).
  • An (RN) Registered Nurse License with no stipulations or restrictions.
  • Basic Life Support (BLS) and ALS skills required or obtain within (30) thirty days if not already held.

  • Comprehensive skills in nursing and patient care standards and procedures.
  • Knowledge of rules, laws, regulations, standards, guidelines, certifying bodies, and accrediting bodies relevant to the department.
  • Knowledge of medical terms, health practices, disease control, medication specs, and side effects.
  • Respect for, and diligent observance of, professional nursing ethics and patient privacy rights.
  • Excellent communication skills, both spoken and written.
  • Basic computer skills: Microsoft Office, information security, scheduling systems, payroll systems, electronic medical records, and email use.

Responsibilities

  • Assess transplant eligibility for patients through interviews with them and their families.
  • Coordinate tests for transplant candidates, identify issues in results, and arrange necessary care.
  • Work closely with patients, physicians, and other healthcare providers.
  • Manage recipient's post-transplant care.
  • Arrange admissions, lab tests, and clinic visits.
  • Coordinate with referring and primary care physicians.
  • Understand test results and identify irregularities.
  • Set up needed care as directed.
  • Teach candidates, families, and caregivers about the evaluation process, organ allocation, waiting period, post-operative care, immunosuppression effects, complications, and psychological impact of transplant.
  • Utilize insurance knowledge to assist with post-transplant prescriptions and coverage issues.
  • Maintain long-term prescriptions.
  • Comply with various regulatory policies and maintain records on the transplant waitlist through different online programs.
  • Coordinate recipient transplants, including using the donor website and communicating with potential recipients.
  • Assess the health status of potential recipients and notify hospital departments.
  • Send updates to the transplant team.
