Requirements

Licensed Occupational Therapist (OT) or license eligible in the state of practice
Graduate of an accredited Occupational Therapist Program
Interest or experience in skilled nursing, long-term care or geriatric rehabilitation
Passion for patient-centered care and improving quality of life for older adults
Willingness to travel to multiple TMC locations as needed

Responsibilities

Evaluate patients and develop personalized care plans focused on functional independence, safety, and quality of life
Deliver care through therapeutic activities, ADL training, and adaptive techniques
Help patients improve their ability to perform daily tasks such as dressing, bathing, grooming, and meal preparation
Quickly integrate with interdisciplinary teams, caregivers, and nursing staff to drive meaningful patient outcomes throughout your assignment
Track patient progress, maintain accurate clinical documentation, and adapt treatment plans based on patient needs and rehabilitation goals
