Requirements

  • Graduate of an accredited Registered Nurse (RN) program.
  • Current and unrestricted registration in good standing with the applicable provincial nursing regulatory body.
  • Current CPR/BLS certification.
  • Minimum one (1) year of recent nursing experience in community health, home care, acute care, primary care, or a related clinical setting preferred.
  • Strong clinical assessment, critical thinking, and care planning skills.
  • Experience with wound care, chronic disease management, medication administration, and patient education is considered an asset.
  • Excellent communication and interpersonal skills, with the ability to build relationships with patients, families, and healthcare partners.
  • Ability to work independently and make sound clinical decisions in community-based settings.
  • Proficiency with electronic documentation and mobile technology.
  • Valid driver's license and access to a reliable vehicle for travel between patient visits.
  • Willingness to travel and work in rural and remote communities, including Manitoulin Island.
  • Demonstrated commitment to providing compassionate, culturally sensitive, and patient-centered care.
  • Flexibility to work a variety of shifts and assignment lengths as required.
  • Proof of vaccination prior to their start date.

Responsibilities

  • Provide comprehensive nursing care to patients in community and home care settings.
  • Conduct patient assessments and develop, implement, and evaluate individualized care plans.
  • Administer medications, treatments, and wound care as prescribed.
  • Monitor patient health status and communicate changes to physicians and members of the healthcare team.
  • Educate patients, families, and caregivers on health management, treatment plans, and self-care techniques.
  • Promote patient independence, safety, and well-being while supporting care in the home environment.
  • Maintain accurate and timely clinical documentation in accordance with professional and organizational standards.
  • Coordinate care and collaborate with interdisciplinary teams to ensure seamless service delivery.
  • Travel between patient homes and community locations as required to provide care.
  • Adhere to infection prevention and control practices, workplace safety standards, and nursing best practices.
  • Build trusting relationships with patients, families, and community partners to support positive health outcomes.
  • Respond effectively to changing patient needs and provide compassionate, patient-centered care in diverse community settings.
