About The Position

We are seeking a compassionate and dependable Licensed Practical Nurse (LPN) for a Temporary to Full-Time opportunity in Buffalo Center, IA. In this role, you will provide essential chronic disease management, routine nursing assessments, and person-centered daily care for residents in a facility-based long-term care setting. You will work closely with RNs, CNAs, physicians, and therapists to maintain our residents' health, safety, and quality of life.

Requirements

• Graduate of an approved Practical Nursing program.
• Active LPN license in the state of Iowa (or a valid compact state license).
• Current Basic Life Support (BLS) / CPR certification.
• Strong communication skills with residents, families, and healthcare team members.

Nice To Haves

• 1+ years of experience in a long-term care or skilled nursing facility.
• Hands-on experience with EHR documentation and care planning.
• Demonstrated knowledge of wound care or palliative care principles.

Responsibilities

• Monitor vital signs, perform routine physical assessments, and report changes in condition to RNs, physicians, and families.
• Safely administer medications, perform wound care, and execute dressing changes per individual care plans.
• Assist residents with activities of daily living (ADLs) and rehabilitation activities as directed.
• Maintain clear, prompt, and accurate records in the Electronic Health Record (EHR) while adhering to facility safety protocols.
• Partner with a multidisciplinary team to implement, refine, and deliver comprehensive care plans.
