(MHC) is a non-profit agency founded in 1993 to serve individuals with severe and persistent mental illness, and has since expanded services to children and adolescents with severe emotional disorders across Middle and East Tennessee. Our sole purpose is to support and treat those challenged with serious mental illness and poverty. Although based out of Nashville, we serve several communities across middle and East Tennessee with satellite offices in Antioch, Gallatin, Dickson, Columbia, Cleveland, Murfreesboro, Clarksville, Cookeville, Chattanooga, and Memphis. This role will assist MHC staff with transportation for their consumers to essential appointments concerning their treatment and recovery. The schedule for this position is Tuesday- Saturday 10AM-6:30pm.

Requirements

High school diploma or general education degree (GED).
Valid Tennessee Driver’s License
Acceptable Motor Vehicle Report (MVR)
Acceptable Criminal Background Investigation
Personal Automobile Insurance
"F" Endorsement
Maintains friendly customer service attitude
Builds appropriate rapport with consumers and teammates
Assists consumers with entering and exiting the vehicle when necessary
Notifies case managers when consumer is a “no-show”
Responds expediently to all communication with office and supervisor
Notifies supervisor of concerns, issues or ideas
Schedules PTO in advance
Reviews route schedule the day before and calls consumers to remind them of their appointment
Consistently gets consumers to appointments on time
Assures minimal wait time when consumer is ready for pick up
On time to all meetings
Assists other drivers when needed
Assists other departments with transports including Crisis, CSU, IIC and the HOST program

Responsibilities

Transport consumers to and from designated appointments.
Transport medications to be delivered to consumers.
Communicate with the team regarding consumer concerns and/or transportation issues for the consumer.
Attend team, department and Agency meetings as required.
Maintain the MHC vehicles in optimal condition and report any concerns to the Transportation Supervisor.
Inspect assigned vehicle at the beginning of work shift and complete vehicle inspection form.
Complete all required paperwork in a legible and timely manner.
Respond expediently to all communication with office.
Manage route, which includes calling consumers prior to leaving the office and planning route effectively.
Attend all mandatory in-services, continuing education and annual health requirements.
Comply with all Corporate Compliance policies, practices and procedures.
Comply with all OSHA and safety policies, practices, and procedures.
Be compliant with the relevant regulations concerning the privacy and security of consumer’s protected health information (PHI) as established by the Health Insurance Portability and Accountability Act of 1996 (HIPAA).
Participate in proactive team efforts to achieve departmental and company goals.
Perform other duties as assigned.
Provide leadership to others through example and sharing of knowledge/skill.
Follow all policies and procedures set by the Agency.
