About The Position

BritePros Healthcare Staffing is seeking a Transplant Program Manager for a role in the Boston, MA area. The candidate will be responsible for maintaining the current network market share and expanding new network opportunities for the Transplant Program. This position is not remote and offers a generous salary of up to $120K, along with benefits.

Requirements

  • BSN required.
  • BLS Certification required.
  • Five or more years of experience in Kidney Transplantation, preferably outpatient experience.
  • Experience with Transplant Medical Records System, EPIC.
  • Quality and Data experience Required.
  • Knowledgeable in CMS and UNOS regulations.
  • Communication Skills: Listens, persuades, and influences others.
  • Leadership skills: Lead by example, Positive thinking, focused on the goal.

Nice To Haves

  • Master’s Degree in Nursing Education preferred.
  • ACLS Certification preferred.
  • CCTC certification preferred.

Responsibilities

  • Act as the single point of contact for all elements associated with new patient referrals for transplant evaluation.
  • Provide information, education, and support to providers and the program’s network of referring providers.
  • Implement systems to recruit new patients and referring providers, improve patient selection, review outcomes, and provide early and accurate feedback to referring clinicians.
  • Track performance data and conduct regular site visits to review results with physicians and staff; create, implement, and follow-up on action plans to further improve patient outcomes and efficiency of the evaluation process.
  • Facilitate collaborative relationships among various disciplines and departments to ensure effective quality patient care delivery.
  • Provide consultation to all new patients and family members regarding the evaluation transplant evaluation process and follow-up requirements.
  • Actively seek new information and maximize creative options to aid in the ongoing learning needs of all members of the referral network.
  • Provide frequent feedback to referring clinicians about the status of their patients within the program.
  • Conduct weekly transplant multi-disciplinary meetings.
  • Facilitate all transplant evaluations and present them at multidisciplinary meetings.
  • Track referral volumes and network relationships.
