About The Position

The Lead Transplant Liaison (Lead TL) is a "player-coach" and operational leader who guides Transplant Liaisons while managing their own patient panel. This role ensures consistent execution of the Transplant Liaison model, reporting to the Transplant Liaison Supervisor. The Lead TL supports teammates through coaching, workflow oversight, and operational guidance to ensure patients receive coordinated support throughout the transplant process. As a subject matter expert, the Lead TL mentors and trains the team on best practices, documentation, and addressing social drivers of health (SDOH). This position helps refine standard operating procedures (SOPs) and workflows to improve program performance and consistency. The Lead TL also supports the supervisor by presenting operational insights to leadership. Key responsibilities include managing workflow, supporting new processes, and addressing complex patient and transplant center challenges. The role uses data to identify performance trends, drive key performance indicators (KPIs), and improve transplant access and outcomes.

Requirements

• Bachelor’s Degree Required
• Minimum 2 years TL experience
• Liaison Program
• Excellent communication and interpersonal skills
• Proficient in computer skills and with proficiency in MS Word, Excel, PowerPoint, and Outlook

Responsibilities

• Provide day-to-day guidance under the supervisor's direction.
• Organize, monitor, evaluate, and support TL workflow for timely and consistent execution.
• Demonstrate the ability to lead a team, delegate tasks, and provide clear work direction.
• Drive program outcomes and key performance metrics by supporting TLs in overcoming barriers and optimizing patient progression.
• Facilitate ongoing teammate development through: coaching, mentoring, and continuous training.
• Attend and/or lead team meetings, trainings, operational calls, and partner sessions as required.
• Demonstrate consistent leadership qualities, professionalism, and reliability.
• Maintain strict confidentiality at all times when handling patient, teammate, and organizational information.
