Responsibilities
- Manage the entire transplant journey using the Nursing Professional Practice Model, overseeing patients from initial referral and evaluation through the inpatient stay and long-term post-transplant transitions.
- Facilitate all aspects of renal and pancreas transplants by scheduling diagnostic testing, collaborating with multidisciplinary teams, and coordinating care between inpatient and outpatient settings.
- Provide expert education and support to transplant recipients, families, and living donors across multiple regions, ensuring they are informed and prepared for the donation and recovery process.
- Oversee all pre- and post-donation activities, including medical testing coordination, family communication, and clinical support in the operating room.
- Conduct inpatient rounds, perform post-operative clinic assessments, and triage patient concerns to ensure long-term health and safety post-transplant.
- Act as a key liaison between patients, referring physicians, and dialysis units, while facilitating multidisciplinary support groups and educational programs.
