About The Position

Sentara Healthcare is seeking a Full Time/Day Shift Transplant Coordinator for our Kidney/Pancreas Transplant team at Sentara Norfolk General Hospital. Sign on bonus available for qualified candidate! Sentara Transplant Center has been around for nearly 40 years and is the only transplant center in Hampton Roads Region. We have performed more than 2,200 transplants. We were the first hospital in the region to become a Magnet® recognized hospital. Sentara Norfolk General Hospital, located in Norfolk, VA, is a 525-bed tertiary care facility that is home to the only Level I Adult Trauma Center and burn trauma unit in Hampton Roads, and also serves as the primary teaching hospital for Eastern Virginia Medical School. In addition to the high-quality heart program at Sentara Heart Hospital, our facility is home to Nightingale Regional Air Ambulance and several other dedicated facilities and specialized services. Requirements

  • Registered Nurse, BSN
  • 2+ years of acute care experience
  • Good medical - renal knowledge
  • Understands chronic disease
  • BLS required
  • Certified Clinical Transplant Coord (CCTC) required within three (3) years of hire.

Responsibilities

  • Coordinate care with post-transplant patients.
  • Facilitates clinic flow, transplant evaluation, and patient care management.
  • Provide weekly education sessions to patients and support member.
  • Arranges appointments with surgeons, social worker, financial coordinator, pharmacy and dietary.
  • Transplant Coordinator admits patient, provides support to patient/caregiver during the waiting period, and discharges transplant patient with instructions for going home.
  • Transplant Coordinator will manage a case load of patients, remain patient’s point of contact through the transplant process, recovery and at home.
