About The Position

The Transplant Coordinator I - Liver Acquisition is a full-time, exempt, union position responsible for developing and maintaining nursing practice standards of care for a specific transplant population. This role serves as a liaison, provides consultation and education, and identifies and resolves clinical issues related to transplant patients. The coordinator collaborates with staff, patients, and families on evaluation and discharge planning, maintains documentation for transplant candidacy and follow-up care, and coordinates communications with the outpatient transplant population. A key responsibility is developing and coordinating a system to manage the patient flow from referral through post-transplant follow-up. The position also involves participating in interdisciplinary research and process improvement activities.

Requirements

  • Nursing BSN
  • One year Transplant Coordinator experience (Heart Transplant, Lung Transplant, Liver Transplant, Kidney/Pancreas Transplant) required.
  • One year Transplant Coordinator experience required for: Heart Transplant - One Year of Cardiology Experience; Lung Transplant - One Year of Pulmonary Experience; Liver Transplant - One Year of Abdominal Transplant Experience; Kidney/Pancreas Transplant - One Year of Abdominal Transplant Experience OR 2 years Organ Procurement experience and 2 years critical care experience required.
  • One year Transplant Coordinator experience (Liver Transplant) required.
  • One year Transplant Coordinator experience required for: Liver Transplant - One Year of Liver Transplant Experience.
  • Minimum of two years of clinical experience in outpatient hepatology or liver disease management required.
  • Excellent communication skills, including the ability to speak, read and write English proficiently.
  • Knowledge of medical procedures and conditions and ability to understand, follow and successfully perform duties, including whole blood collection, and apheresis procedures, in accordance with administrative and clinical policies, regulations and procedures.
  • Active CA RN license
  • Basic Life Support (BLS) Healthcare Provider from American Heart Association
  • Fire Life Safety Training (LA City) - If no card upon hire, one must be obtained within 30 days of hire and maintained by renewal before expiration date. (Required within LA City only)

Nice To Haves

  • Nursing Master’s Degree in Nursing or in progress, strongly preferred.
  • Master’s Degree in Nursing may be in lieu-of Bachelor’s Degree in Nursing.
  • Excellent communication skills in Spanish, including the ability to speak proficiently.

Responsibilities

  • Independent coordination of outpatient care of the transplant patient by participating in clinic visits, analyzing diagnostic tests, monitoring patient and family compliance, reviewing data and initiating appropriate action, and identifying and intervening in patient care problems.
  • Coordinates donor procurement and recipient preparation by maintaining the UNOS waiting list, communicating with transplant team members about donor and recipient status, and coordinating the admission process and preoperative preparation.
  • Participates in the development of policies and procedures pertinent to the transplant program.
  • Participates in and develops appropriate educational programs for patients, families, nursing staff, and medical staff.
  • Serves as a resource person to all staff involved in the care of transplant patients and their families.
  • Communicates identified patient and family problems to multidisciplinary team members and assists with interventions.
  • Keeps abreast of regulatory Hospital, CMS, OPTN/UNOS policies, and ongoing clinical education in transplant and evidenced-based research.
  • Independently facilitates the evaluation and listing process of potential transplant patients within the regulatory requirements of CMS and OPTN/UNOS.
  • Maintains communication with referring physicians and insurance case managers, supplying necessary information and records, and providing education regarding the transplant process.
  • Database management, including maintaining the UNOS waiting list, completing TIEDI Forms, managing internal databases, and collecting data during the transplant process.
  • Oversees assignments delegated to support staff, evaluates their performance, and provides direct supervision.
  • Functions independently within clinical skills and established processes for patient flow.
  • Works in collaboration with the multidisciplinary transplant team to provide care for transplant patients across the continuum.
  • Attends department quality meetings and participates in QAPI activities.
  • Adheres to all hospital policies including attendance, punctuality, and dress code.
  • Maintains a professional and positive attitude at all times in both patient and office settings.
  • Performs other duties as assigned.
