Facilitates effective clinic organization and patient care by serving as a communication link between the patients, staff, physicians, hospital departments, or other outside health care entities. Provides care to and related activities for designated patients while following hospital and department-specific policies and procedures. Coordinates care for pre-transplant, post-transplant, living donor, and/or surgical patients. Reviews medical history and suitability of patients; coordinates scheduling of appropriate medical testing and reviews test results. Coordinates and/or participates in the education of transplant and/or surgical patients. Coordinates organ transplant with Organ Procurement Organization, transplant candidate, physicians, and hospital staff. Develops and maintains systems for data management to meet requirements. Monitors patient lab results in OTTR, transplant's legal medical record. Maintains an up-to-date transplant waiting list by coordinating patient's activities with the nursing staff. Maintains contact with transplant candidates to ensure compliance with a care plan. Monitors and audits patients' charts to ensure that they are compliant with both CMS and UNOS regulations and requirements. Monitors patients' cPRA results in Histotrac to assure their transplant readiness and appropriateness of donors Reviews referral information from nephrologists, dialysis units, endocrinologists, or other referral sources.
