About The Position

The Transitions of Care (TOC) Team supports members who are newly discharged from the hospital and, in some cases, before discharge. The role advocates for members by coordinating care and ensuring they have the resources and support needed to recover safely at home and avoid hospital readmissions. The ideal candidate is caring, compassionate, energetic, and enjoys connecting with people.

Requirements

• Active and clear Medical Assistant Certification.
• 2–3 years of experience in healthcare, managed care, home health, and/or a community health setting.

Nice To Haves

• Fully bilingual in English and Spanish.

Responsibilities

• Call patients inbound and outbound after discharge to check on their well-being and confirm they have received needed assistance.
• Coordinate member services and care with affiliated vendors, including transportation, durable medical equipment, meals, and home health care, as appropriate.
• Assist with scheduling post-discharge TOC APP, PCP, and specialist visits.
• Collaborate with patients, providers, and caregivers to support positive outcomes.
• Work with other TOC team members to address patient needs and meet team-based goals, including through interdisciplinary huddles.
• Participate in and assist with pilot programs.
• Coordinate with medical providers and facilities to obtain necessary discharge records.
• Follow escalation processes to the clinical team to help ensure patients receive needed evaluation and care.
