About The Position

Transitions of Care (TOC) is one of Devoted Medical's largest clinical programs, responsible for making sure thousands of Devoted members get home safely after a hospitalization. From admission through 30 days post-discharge, our virtual interdisciplinary teams deliver personalized, comprehensive wrap-around care that breaks down the clinical, social, and logistical barriers to post-discharge stability. As a TOC RN Manager, you'll lead and support a team of RN Case Managers, shaping culture, performance, and clinical excellence by mentoring nurses, supporting their clinical decision-making, and making sure care is delivered with compassion, integrity, and skill. You'll be part of our interdisciplinary leadership team — alongside peer RN Managers and leaders from our Nurse Practitioner, Care Coordinator, and Associate disciplines — collaborating on day-to-day operations, patient care strategy, and the continuous improvement of our virtual care model. Requirements

  • Active RN license
  • Minimum 2 years of direct patient care experience in care management, population health, or transitions of care
  • Minimum 2 years of people management experience, with a track record of team development and performance oversight
  • Experience managing licensed care management teams
  • Proven ability to drive clinical and operational outcomes through new initiatives and data-informed strategies
  • Comfort reviewing, interpreting, and acting on performance data
  • Strong communication and collaboration skills, with a can-do attitude and a learning mindset

Nice To Haves

  • Compact/multi-state RN licensure
  • Experience delivering care management in a virtual or telehealth setting
  • Experience coaching and educating clinical staff, creating a learning-rich environment where nurses grow in confidence and capability
  • Highly collaborative, with a track record of breaking down silos and embracing interdisciplinary partnership

Responsibilities

  • Coaching and supporting RN Case Managers through 1:1 meetings, shadowing, audit reviews, and time studies
  • Supporting RN Case Managers with escalated or complex patient needs, actively participating in clinical huddles, and maintaining a consistent presence in patient outreach — dedicating at least 10% of your time monthly to direct member engagement
  • Designing and implementing workflows, assessments, and protocols that improve efficiency and demonstrate patient impact
  • Identifying workflow challenges, contributing to cross-functional problem-solving, and helping standardize best practices across teams
  • Developing onboarding and ongoing education experiences for new and existing staff
  • Collaborating with cross-functional leaders to align and optimize care coordination across programs
  • Meeting regularly with the Senior Manager of TOC to review performance metrics, surface risks, and celebrate team successes
