About The Position

The Transitions Care Coordinator serves to facilitate a seamless transition for patients transitioning from a facility to the Home care environment along with executing the sales strategy to increase company market share through account development and educating the medical community on our services. After patient has selected NHC HomeCare as his or her health care provider, the Transitions Care Coordinator will assist patients in the process of navigating their post-acute care needs. The Transitions Care Coordinator will develop strategic plans to successfully manage and grow accounts and market share. Grow market share by obtaining referrals and increased participation in the NHC program. Represents NHC Homecare in a positive and professional manner, projecting and reinforcing company objectives and philosophy. Requirements

  • One-year home health experience or one year of hospital case management experience.
  • Successful previous experience in business development, outreach, or healthcare related industry.
  • Reliable means of transportation.
  • Must have current driver’s license and auto insurance.

Nice To Haves

  • Current BSW, MSW, LPN or RN license in the state of service.
  • 1-2 years Experience in Home Health and / or healthcare business development.

Responsibilities

  • Facilitate a seamless transition for patients from a facility to the Home care environment.
  • Execute the sales strategy to increase company market share through account development.
  • Educate the medical community on our services.
  • Assist patients in navigating their post-acute care needs after they have selected NHC HomeCare.
  • Assess, plan, implement, coordinate, monitor and evaluate options and services with a primary goal of providing a safe transition from a facility to home for home health.
  • Ensure the patient has a physician and obtains an order from that physician to oversee the home health plan of care.
  • Obtain and communicate Face to Face documentation to the appropriate agency.
  • Develop strategic plans to successfully manage and grow accounts and market share.
  • Grow market share by obtaining referrals and increased participation in the NHC program.
  • Communicate effectively with all members of the health care delivery team.
  • Represent NHC Homecare in a positive and professional manner, projecting and reinforcing company objectives and philosophy.
