As a Transitional Recovery Support Coach at Horizon, you will… Help to initiate and sustain an individual/family in recovery from substance use or addiction and promote recovery by removing barriers and obstacles to recovery. If needed through transitions of care, help a patient find resources for harm reduction, detox, treatment, family support, and education/employment, local or online support groups, or help a patient create a change plan to recover on their own. Help individuals find ways to stop using (abstinence), or reduce harm associated with addictive behaviors. Complete data reporting/documentation on linkages to services for individuals post-discharge from residential treatment. Monitor outcomes for treatment engagement and social support linkage. Facilitate weekly orientation of services and group therapy sessions in recovery management.

Requirements

  • Associate's degree in human services, Social Work, or other related field along with 1 year of experience working as a Recovery Coach or in a similar role required.
  • OR High school diploma with OASAS Certified Peer Advocate Certification or OMH Certified Peer Specialist and four years of experience working as a Recovery Coach or in a similar role required.
  • Excellent communication skills
  • Excellent computer skills
  • Reliable transportation

Nice To Haves

  • Some experience with the SUD population in residential setting and high-risk populations preferred
