The Youth and Young Adult Transitional Housing Case Manager provides intensive, strengths-based case management for young people, ages 18–24 experiencing homelessness and residing in One80 Place transitional housing program. This role helps youth achieve stability and self-sufficiency by coordinating housing supports, life-skills development, education and employment pathways, and access to community resources. The Case Manager builds trusting, trauma-informed relationships that empower youth to set and achieve housing stability goals. This position may work with young adults who have exited the Department of Juvenile Justice, the foster care system or pregnant or parenting youth, and all other populations of youth experiencing homelessness. The Transitional Housing Case Manager will work in a combination of settings – in office, community based, and in One80 Place’s Transitional Housing and emergency shelter locations. This position may require evening or occasional weekend hours based on youth needs. This position is funded in whole by One80 Place’s Youth Homelessness Demonstration Program grant. One80 Place ends and prevents homelessness throughout South Carolina with offices located in Charleston and Columbia. This position will be based in Charleston.
