About The Position

We are seeking a compassionate and detail-oriented Transitional Care Technician to support patients as they transition from hospital to home or other care settings, helping ensure a smooth and safe process. The role includes assisting with daily activities and mobility, providing emotional support and post-discharge care education, coordinating care with healthcare professionals, monitoring patient conditions, performing basic medical tasks, documenting care, and maintaining a clean, safe, and comfortable environment. Requirements

High school diploma or equivalent.
Strong communication and interpersonal skills.
Compassionate, patient-focused approach and ability to handle sensitive situations.

Flexibility to work various shifts, including evenings and weekends.

Nice To Haves

Responsibilities
