About The Position

BAYADA Home Health Care is seeking a compassionate, driven, and relationship-focused Transitional Care Manager to support the ERM market. This is a non-clinical role focused on building referral relationships, connecting clients and families with home health services, and supporting successful transitions from hospitals and other health care settings to care at home. Requirements

  • This is a non-clinical position; a clinical license is not required.
  • At least three years of experience in sales, marketing, community relations, health care business development, referral coordination, customer service, or a similar role.
  • Demonstrated record of strong interpersonal, communication, and relationship-building skills.
  • Ability to build credibility and establish productive relationships with referral sources and health care professionals.
  • Strong organization, follow-through, and time-management skills.
  • Ability to work independently, manage priorities, and collaborate effectively across teams.
  • Willingness and ability to travel regularly throughout the assigned ERM market.

  • Previous experience working with hospitals, rehabilitation facilities, physician practices, discharge planners, or community-based health care partners preferred.

Responsibilities

  • Develop and maintain strong professional relationships, in accordance with The BAYADA Way, with Ennoble Care stakeholders and partners directly connected to the Ennoble partnership and referral pathway.
  • Serve as a central point of coordination for the two-way Ennoble–BAYADA referral pathway, supporting referrals from Ennoble to BAYADA home health as well as referrals from BAYADA to Ennoble Care.
  • Drive referral conversion and timely follow-through by monitoring referrals through the scheduling and care-initiation process, identifying barriers or delays, and coordinating with the appropriate Ennoble and BAYADA teams to help move referrals forward.
  • Manage and review referral pending reports daily, proactively following up on outstanding referrals, escalating barriers when appropriate, and helping ensure referrals are scheduled, completed, or otherwise appropriately resolved in a timely manner.
  • Help maintain visibility into referral status across the partnership and close the communication loop with appropriate Ennoble and BAYADA stakeholders when referrals are delayed, unable to be scheduled, or require additional action.
  • Build trust and engagement with internal Ennoble champions and external partners involved in supporting the Ennoble partnership, while maintaining clear alignment with BAYADA’s local sales teams and established referral-source relationships.
  • Develop a strong understanding of BAYADA’s home health services, Ennoble’s care model, the Ennoble–BAYADA referral processes, and available resources in order to effectively support both sides of the partnership.
  • Identify opportunities to strengthen the Ennoble–BAYADA referral pathway by recognizing trends, identifying operational barriers, facilitating solutions, and supporting consistent processes across markets.
  • Gather and communicate referral information accurately to the appropriate BAYADA and Ennoble teams.
  • Coordinate the non-clinical aspects of referrals and facilitate timely communication among Ennoble teams, BAYADA service offices, clinical teams, clients, and families as appropriate.
  • Maintain accurate referral information, follow-up activity, pending status, and required documentation related to the Ennoble partnership.
  • Partner closely with BAYADA local sales leadership and assigned marketers, ensuring Ennoble TCM activities complement — rather than duplicate — existing market strategies and established referral-source relationships.
  • Collaborate with fellow Ennoble Transitional Care Managers, BAYADA sales and clinical leaders, and service-office teams to identify barriers, share best practices, and support responsible growth of the Ennoble partnership.
  • Represent BAYADA professionally in interactions and activities specifically associated with the Ennoble Care partnership.
