Position Summary: Build and strengthen relationships between referral sources and the company. Focus on relationships with hospital and facility contacts to educate their leaders about company services and the benefits of home health care. Use clinical knowledge to educate patients about home health service eligibility and expectations. Manage referral intake and coordinate the initial transition to home care. This is hybrid position, that will require travel throughout Atlantic County, NJ.
