About The Position

The Transitional Care Coordinator manages cross-continuum care episodes and provides ongoing assessments of patient and family needs via telephonic and in-person interactions. Requirements

• A graduate of an accredited school of nursing.
• Prior to date of hire, must hold a current, valid RN license or temporary RN license, or hold a compact RN license. Must maintain valid licensure to practice as a RN.
• CPR required
• Minimum of three (3) years’ experience in a healthcare setting required.
• Must have experience with medical case management.
• Valid driver's license with maintenance of safe driving record and an automobile that is insured in accordance with state requirements required.

Responsibilities

• Identify opportunities to improve the patient and family experience of care and to improve the efficiency and effectiveness of resource use.
• Encourage patient and family engagement with disease and self-management through identification of new and existing barriers.
• Assist with process improvement, care integration, and a primary focus on reducing hospital readmissions.
• Consult and educates the patient/family and other caregivers regarding the disease process, pain and symptom management, end of life care and processes when appropriate.
