The Transition To Practice Facilitator coordinates the orientation of new staff members to nursing inpatient units. They partner with preceptors, CTLs, Unit Director and Clinical Unit Educator to ensure that all staff are clinically competent. This role also coordinates the orientation of new employees to nursing inpatient units for LPN and Ancillary positions. The facilitator assesses the needs of the new employee and partners with preceptors, CTLs, Unit Director, Clinical Unit Educator, and the Education and Organizational Development department to provide education based on learning assessment, observations, and feedback of staff members on the units. Additionally, they coordinate preceptor/assistive staff assignments for new employees, develop an orientation calendar for each new employee (including classes, orientation shifts, and preceptor names), and ensure the appropriate scheduling of live and online classes. The role involves developing and following up on employee goals, providing a positive learning environment, and ensuring a smooth transition for transfer employees and leaders. They partner with the unit to ensure new hires are welcomed and acclimated to the unit culture, and collaborate with the unit director and preceptor to assign a mentor to all licensed staff for the first year after orientation. The facilitator greets new hires, ensures successful onboarding, meets informally with new licensed staff every two weeks, and collaborates with the preceptor to schedule bi-weekly meetings with new hires and the unit director. They are responsible for monitoring and tracking orientation status, intervening with action plans for those not on track, and making recommendations for revising unit orientation plans based on feedback. Collaboration with the Nursing Onboarding Manager on training, orientation, and recruitment is key. The role identifies and monitors orientation and continuing education requirements for compliance, assists with Preceptor Development, and serves as a resource for preceptors. They develop relationships with unit preceptors and are a member of the Nursing Education Council, ensuring off-shift and weekend staff are supported and coordinating regular communication with new hires. The facilitator acts as a liaison between preceptor/manager and new hire to resolve learning/adjustment difficulties. They assess and monitor the progress of new graduate RNs during their first year, initiate peer support groups, make referrals to Career Planning and Transition, and conduct surveys. Finally, they observe new hires performing in their role, validate competencies, shadow those not meeting milestones to understand learning needs, participate in unit in-services and clinical education sessions, and develop a work schedule that provides visibility and rounding on all assigned units, using discretion and independent judgment daily.
