About The Position

The Transition Support Staff at ARC Human Services is responsible for facilitating and supporting individuals through significant life changes. This role is crucial in providing guidance, resources, and emotional support to ensure a smooth transition for clients, whether they are moving into new living arrangements, starting new employment, or adjusting to other life changes. The Transition Support Staff plays a pivotal role in empowering clients to achieve independence and improve their quality of life.

Requirements

• High school diploma or equivalent required
• Minimum of 1 year of experience in a support or caregiving role, preferably within a human services or social work environment.
• Proven track record of effectively managing multiple tasks and priorities in a fast-paced setting.
• Strong interpersonal and communication skills, with the ability to interact compassionately and professionally with clients, families, and team members.
• Demonstrated ability to work independently as well as collaboratively within a team.
• Excellent problem-solving skills and the ability to adapt to changing circumstances.
• Proficiency in basic computer applications, including Microsoft Office Suite.
• Ability to maintain confidentiality and adhere to ethical guidelines in all interactions.
• Valid driver's license and reliable transportation may be required for certain positions.

Nice To Haves

• associate degree in human services or related field preferred.
• Experience working with individuals with disabilities or special needs is highly desirable.

Responsibilities

• Assist clients in developing personalized transition plans, setting realistic goals, and identifying necessary resources.
• Provide emotional and practical support to clients during their transition process, ensuring they feel supported and understood.
• Collaborate with other service providers, professionals, and community resources to coordinate comprehensive support for clients.
• Monitor and evaluate the progress of clients, making adjustments to transition plans as needed to ensure successful outcomes.
• Maintain accurate and up-to-date records of client interactions, progress, and any changes in their circumstances.
• Advocate for clients' needs and rights, ensuring they have access to necessary services and opportunities.
• Facilitate workshops or group sessions to provide clients with skills and knowledge beneficial for their transition.
• Stay informed about best practices and emerging trends in transition support to continually enhance service delivery.
• Uphold ARC Human Services’ mission and values in all interactions with clients, colleagues, and external partners.
