About The Position

Patient Engagement Advisors (PEA) is seeking a full-time Transition Specialist to work onsite at AdventHealth Tampa. In this role, you will set patients up for success as they transition out of the hospital into their next stage of care. You will interact face-to-face with patients, work collaboratively as a critical part of the clinical care team, and have unique career path options for growth and development. Requirements

• High School Education or equivalent required
• 6 months pharmacy technician (retail preferred) OR 1-year medical assistant / patient care technician / patient care coordinator / clinic front desk associate / nursing assistant / home health aide / other healthcare or medical experience required
• Other related healthcare, hospital, or medical experience will be considered
• Ability to report to work onsite as scheduled
• Ability to be on feet – standing, walking, and/or climbing stairs 8-10 hours daily
• Ability to receive required vaccinations (as required by PEA or hospital) unless approved for exemption

Responsibilities

• Schedule patient’s follow-up appointments
• Schedule patient’s transportation as needed
• Deliver discharge prescription medications to patient’s bedside
• Assist patients with medication affordability
• Coordinate home meal delivery service for patients
• Connect patients to community resources as needed
