About The Position
We are seeking a Transition Specialist to support youth transitioning from juvenile correctional facilities back into their communities.
Nice To Haves
- Experience in youth services, case management, social work, juvenile justice, corrections, counseling, or community outreach.
- Strong communication, organizational, and relationship-building skills.
- Experience working with at-risk youth or reentry programs is highly preferred.
Responsibilities
- Develop and coordinate youth transition and reentry plans.
- Teach life skills and support educational goals.
- Help youth obtain IDs, Social Security cards, birth certificates, and other required documents.
- Assist with returning to school, employment, apprenticeships, or workforce programs.
- Coordinate with schools, families, community agencies, and state organizations.
- Conduct follow-up calls after youth are released.
- Participate in transition planning meetings and represent the program at community events.
