Provides support with identifying, overseeing, and managing the coordination of transition of members in the community. Works with leadership to ensure the timely and safe transition of members in the community from various levels of health care services including coordinating care plans with community care coordinators, educating transition enrollees about services, requirements, limitations, and/or exclusions of services as a result of the transition. May perform and/or assist with member assessment/screenings; may develop and/or assist with developing member transition plan or service plan/care plan. This position works Monday through Friday, 8:00 a.m. to 5:00 p.m. ET. Candidates must reside in New Jersey, with a preference for those located in Northern New Jersey, and be comfortable traveling regularly to nursing facilities and other community locations. The role supports members transitioning from healthcare facilities into community-based settings by coordinating with care teams, providers, members, families, and caregivers; arranging essential household items, furnishings, groceries, and in-home setup; addressing barriers to safe community placement; and maintaining accurate documentation, tracking, and monthly reporting.
