About The Position

We are seeking a compassionate, organized, and collaborative Day Treatment Transition Specialist to join our multidisciplinary Day Treatment team. In this role, you will serve as a key liaison between the Day Treatment Program, local school districts, mental health providers, and families to support youth transitioning from higher levels of care back into community school settings. This position plays a critical role in coordinating educational, behavioral, and mental health supports to ensure successful reintegration and continuity of care for clients. The ideal candidate is passionate about youth mental health, skilled in interdisciplinary collaboration, and experienced working with schools and community-based systems. The Day Treatment Transition Specialist works closely with therapists, educators, school districts, and treatment team members to coordinate discharge planning and transition services for youth returning to their local schools after participation in the Day Treatment Program. This position ensures communication, coordination, and support systems are in place to address each client’s educational, behavioral, physical health, and mental health needs during transition into a lower level of care.

Requirements

  • Understanding of child and adolescent development, treatment standards, and behavioral health models within a multidisciplinary setting.
  • Ability to professionally represent the agency and effectively coordinate care with schools, community agencies, and external providers.
  • Strong skills in interdisciplinary communication, treatment planning, engagement, documentation, and care coordination.
  • Bachelor’s degree in behavioral sciences or a related field, or equivalent combination of education and experience required
  • Two (2) years of direct care experience working with at-risk children and/or adolescents preferred.

Responsibilities

  • Coordinate with home school districts regarding transportation needs at the time of admission.
  • Begin discharge planning and school placement coordination within the first week of client admission.
  • Serve as a liaison between the Day Treatment Program and local school districts.
  • Build and maintain positive working relationships with school district personnel to support successful student transitions.
  • Collaborate with schools to support reintegration plans that address educational, behavioral, and mental health needs.
  • Ensure school district representation in treatment planning meetings and ongoing treatment team collaboration.
  • Consult with teachers and school administrators regarding behavioral and educational supports for transitioning students.
  • Work closely with Day Treatment Therapists beginning at intake to coordinate post-discharge mental health services.
  • Ensure receiving schools are connected with discharge mental health providers to support continuity of care.
  • Promote the Day Treatment Program and stabilization model with school districts and funding partners, including Medicaid and private insurance providers.
  • Collaborate with therapists, psychologists, educators, and other treatment team members regarding treatment concerns and discharge planning.
  • Administer medications to clients as needed and according to agency policy.
