The Arc of Ocean County is looking for a compassionate and motivated Transition Services Specialist to help adults with intellectual and developmental disabilities prepare for successful employment, greater independence, and active participation in their communities. Requirements

  • Bachelor's degree in Human Services, Psychology, Social Work, Rehabilitation Services, Special Education, or a closely related field.
  • Valid New Jersey driver's license with a satisfactory driving record (five points or fewer).
  • Excellent communication, organizational, and relationship-building skills.
  • Ability to work independently while collaborating effectively with coworkers, families, employers, and community partners.
  • Flexibility to participate in community-based programming throughout Ocean County.
  • Ability to lift and carry at least 25 pounds when necessary.
  • Ability to assist individuals with mobility-related needs.
  • Ability to travel throughout the community and actively participate in off-site programming.

Responsibilities

  • Deliver engaging instruction that strengthens employment readiness, communication, independent living, and interpersonal skills.
  • Design activities that encourage confidence, responsibility, and personal achievement.
  • Adapt teaching methods to meet each individual's strengths, learning style, and support needs.
  • Provide ongoing encouragement that promotes self-advocacy, independence, and personal growth.
  • Support participants during volunteer experiences, career exploration activities, and community-based learning opportunities.
  • Coach individuals on workplace expectations, professionalism, and effective communication in real-world environments.
  • Develop positive relationships with local businesses, employers, and community partners to expand opportunities for participants.
  • Safely transport individuals using agency vehicles to program sites and community activities.
  • Assist individuals in working toward goals identified in their Individual Service Plans (ISPs).
  • Participate in interdisciplinary meetings and provide meaningful updates regarding participant progress.
  • Maintain timely and accurate documentation, including daily service records, progress notes, medication administration, and incident reports.
  • Utilize positive behavior support strategies while responding professionally to changing participant needs.
  • Administer medications in accordance with agency policy and applicable regulations.
  • Follow individualized health, behavioral, nutritional, and adaptive support plans.
  • Foster an environment built on dignity, respect, inclusion, and personal choice.
  • Ensure compliance with agency standards, safety procedures, and program requirements.
