About The Position

Achievement Centers for Children is a Cleveland based nonprofit organization dedicated to helping children and young adults with disabilities and their families achieve and thrive. Through innovative education, therapy, recreation, and family support services, we serve individuals across Northeast Ohio with a commitment to excellence, dignity, and inclusion. Our Autism School delivers specialized, evidence based educational programming designed to meet the unique needs of students with autism. We combine clinical expertise, structured intervention methodologies, and strong family partnerships to create meaningful outcomes for every student we serve. As part of our continued growth and strategic evolution, we are strengthening our transition programming to expand community based instruction, deepen agency partnerships, and improve post school outcomes for middle and high school students.

Requirements

  • Bachelors degree in a related field that supports the acquisition of a Transition to Work endorsement
  • Transition to Work endorsement or documented enrollment in an approved program leading to the endorsement
  • At least three years experience working in approved programs for students or young adults with disabilities, with at least one year at the high school level or equivalent experience
  • Working knowledge of Ohio Department of Education transition services requirements and Academic Content Standards for grades K through 12 plus, including Extended Standards
  • Demonstrated ability to collaborate successfully with community based agencies and build partnerships
  • Clear criminal background as required
  • Valid Ohio drivers license, required auto insurance, and an acceptable driving record
  • Strong written and verbal communication skills
  • Ability to relate positively and professionally with students, families, and staff
  • Ability to instruct and support staff on evidence based practices for students with autism
  • Strong organizational skills and the ability to manage multiple priorities
  • Ability to work independently with sound judgment, initiative, and professionalism
  • Strong problem solving skills, including in high stress situations
  • Ability to maintain confidentiality and act with integrity
  • Proficiency in Microsoft applications and technology required to perform the role
  • Ability to travel for community based instruction and supervision activities
  • Ability to use a computer and complete documentation requirements
  • Ability to support safe and effective learning environments for students and staff

Responsibilities

  • Assess, design, coordinate, and implement transition services for middle and high school students with autism
  • Develop and facilitate community based instruction experiences aligned with student IEP transition goals
  • Ensure compliance with Ohio Department of Education transition services requirements and Academic Content Standards, including Extended Standards
  • Partner with students, parents and guardians, and school staff to support post secondary planning, independence, and workforce readiness
  • Build and maintain partnerships with community based agencies, service providers, and employers
  • Provide consultation, staff education, and professional learning opportunities related to transition planning and evidence based practices that support students with autism
  • Maintain accurate documentation and communication required for coordination, compliance, and program effectiveness
  • Promote a safe, organized, and healthy learning environment, including adherence to infection control and universal precautions
  • Represent the Autism School and Achievement Centers for Children in a knowledgeable, courteous, and professional manner
