About The Position

Rite of Passage Team is hiring for a Transition Services Coordinator at Les Peters Youth Academy in Tampa, Florida. The program is a moderate-risk residential commitment program for female youth aged 14 to 19, providing comprehensive care, treatment, supervision, and evidence-based intensive mental health services, including individual, family, and group counseling. The company is seeking applicants from the St. Petersburg, FL area as the site will be relocating. Requirements

• Have a bachelor's degree or better.
• Be able to pass a criminal background check, drug screening, physical, and TB Test.

Responsibilities

• Supervises a caseload of approximately 14-24 students.
• Assists with facilitating court hearings, Probation Officer visits, student visits, and/or home passes.
• Meets with students to review progress.
• Serves as the primary program liaison between student, parents/legal guardian, and placing agency officials.
• Provides monthly progress notes and weekly student updates.
• Coordinates activities with other program staff.
• Participates in assessments and treatment plans.
