About The Position

The Special Education Transition Coordinator collaborates with students, families, school staff, and community agencies to develop and implement transition services that prepare students with disabilities for post-secondary education, employment, and independent living. This position oversees the district's transition program, supports the development of transition-focused IEPs, coordinates community partnerships and agency referrals, and ensures students have access to meaningful transition opportunities.

Requirements

Must hold a valid Connecticut State Department of Education (CSDE) educator certificate with an endorsement in Comprehensive Special Education (065, 165, or 265)
Three or more years of experience in Secondary Special Education and/or Vocational Transition Education/Services
Demonstrated experience in writing standards-based IEPs, including goals and objectives for transition planning
Demonstrated knowledge of Connecticut Core Transition Skills
Demonstrated experience in aligning curriculum with Connecticut Standards for Transition Planning
Demonstrated experience developing programming for transition planning in post-secondary education, employment, and independent living
Demonstrated ability to evaluate individuals for transition planning, systems and/or working with public schools to assess needs of special needs students
Demonstrated knowledge of best practice for the education of students for transition

Nice To Haves

Bilingual proficiency in Spanish or Haitian Creole highly desirable.

Responsibilities

Conduct vocational and transition assessments, review student records, and develop person-centered plans that support individualized post-secondary goals.
Coordinate transition services, community-based learning, employment opportunities, and independent living experiences aligned with students' IEPs.
Build partnerships with families, employers, community organizations, and state agencies to facilitate referrals, resources, and successful student transitions.
Develop and implement transition programming, including career readiness, self-determination, functional life skills, and transportation training, while supporting job coaches and monitoring student progress.
Provide training and guidance to staff, students, and families on transition planning, available services, legal requirements, and best practices to ensure successful post-school outcomes.
