About The Position

The Transition Coordinator will facilitate the implementation of a life skills curriculum, titled Next Steps, and utilize the Transition Coordinator Handbook to standardize transition practices. This role involves evaluating transition consultations, participating in Administrative Release Committees (ARC) to confirm reentry plans, and conducting follow-up phone calls to released youth at various intervals. The Transition Coordinator will also represent client services at conferences and meetings, assist in developing partnerships, and contribute to the creation of individualized Education Learning Plans. Additionally, the role requires working with state, county, and private agencies to provide re-entry services, assisting students with obtaining necessary documentation (social security cards, state IDs, birth certificates, etc.), and supporting their transition back to public schools, employment, apprenticeship programs, or military service.

Requirements

  • Must have a valid driver's license.
  • Must have reliable transportation.
  • Must have a high school diploma or GED.
  • Must be able to pass a background check.

Responsibilities

  • Facilitate the implementation of a life skills curriculum, titled Next Steps.
  • Utilize the Transition Coordinator Handbook in an effort to standardize the transition practices.
  • Evaluate the five "Transition Consultations" to be completed by every student under the guidance of an assigned teacher.
  • Participate on a weekly basis in Administrative Release Committees (ARC) in each facility to confirm and reinforce the student's reentry plans.
  • Do the Follow Up Phone Calls to released youth at 1, 3, 6, 9, and 12 months.
  • Represent the client Services at many conferences and meetings.
  • Assist in the development of a partnership.
  • Assist in the development of the individualized Education Learning Plan for each youth upon entry at the facility.
  • Work with state, county and private agencies to provide re-entry servicers for youth transitioning back into the community.
  • Assist students with obtaining social security cards, state identification, driver’s license, birth certificates, and additional documentation.
  • Assist students with transitioning back to public schools.
  • Assist students with employment, apprenticeship programs, and military service information.
