The Transition Coordinator supports the school's mission and vision by collaborating with various stakeholders to implement new and existing curricula, make recommendations for program improvement, and support teachers in enhancing instructional practices. This role is crucial in fostering a culture of lifelong learning and reflective practice. A key responsibility is coordinating the vocational training program, including developing and managing school and community-based work experiences, and acting as the primary liaison between the school and worksites. The coordinator will also oversee van usage for trips, manage vehicle maintenance, and lead the transition department, including supervising and training job coaches. They will guide the development and implementation of in-service training programs for the transition department, complete essential transition planning forms, and collaborate with the IEP Team to create IEP objectives aligned with students' postsecondary goals. Participation in various team meetings, consultation with teachers to ensure community trips align with IEP goals, and planning/maintaining a transition curriculum are also core duties. The role involves administering vocational and transition assessments, providing direct training in independent living and community access, and coordinating services within the community. Collaboration with districts, state agencies, and local service providers is essential for seamless transitions to adult services. The coordinator will also manage student participation in specific initiatives, assist teachers with transition portfolios and MyCAP activities, and collect data on post-school outcomes. Classroom coverage and other assigned tasks are also part of the role.
