About The Position

The Transition Coach for the Youth Empowerment and Support Transition Program has primary responsibility for coordination of services for young people in foster care and alumni of foster care. The Transition Coach works intensely with the youth and host family by providing ongoing support and will help to build a plan of independence with the youth they are serving in the program. The Transition Coach works closely with the Program Supervisor and Case worker to provide high-quality intervention and transition support services for transition age youth ages 16 to 21 - not only to empower youth to develop concrete, achievable success plans, but also to assist youth in achieving independence, housing stability, education, employment, and community connectedness.

Requirements

  • High School Diploma or GED required
  • Personally lived experience or work experience in child welfare required
  • Driver's license and ability to maintain insurability throughout employment required

Responsibilities

  • Provides support to ensure youth remain in the community by increasing attachments to school, community, and family.
  • Identifies natural supports; increases supportive networks, independent life skills, and engagement in mental health, substance abuse treatment, and pro-social activities.
  • Maintain understanding of how Social Determinants of Health (SDOH) impact the youth's quality-of-life outcomes and risks.
  • Support youth in finding and maintaining Housing by assisting in locating income based or voucher approved housing for youth; Assist youth with application process, lease signings, securing furniture, and the move in process.
  • Provide emotional and crisis support as needed to clients by engaging in face-to-face, phone, text and email correspondence with young adults to discuss needs in relation to meeting goals.
  • Assist youth with community resources and referrals to achieve goals.
  • Assist youth with preparedness/readiness for their goal as needed (i.e., mock interviewing, resume building, job applications, college applications, FAFSA and other college documents, etc.)
  • Attend court hearings and provide transportation to clients as it pertains to meetings or maintaining Goal Plan.
  • Demonstrate strong leadership with the ability to motivate and inspire a diverse group of volunteers and interns.
  • Assist client with community resources for vehicles, car maintenance and management.
  • Collaborate with youth, case workers, and program supervisor to ensure needs and goals are met.
  • Act as role model for youth in program.
