About The Position

Requirements
- Valid LVN license in State of Texas.
- Previous experience in behavioral health and substance use field.
- Working with case management, hospitals, and admissions preferred.
- Excellent customer services, verbal and written communication skills.
- Organizational and multitasking skills.
Responsibilities
- Providing high quality, prompt assistance with triaging and coordination of transfers from other facilities.
- Monitoring and responding to triage needs and completing direct referral acceptance process.
- Using technology as a platform for reviewing patient’s clinical needs and delivering prompt communication of acceptance or deflection.
- Collaborating and working closely with referral sources, physician’s, nurse’s, etc.
