About The Position

Presbyterian is hiring a skilled Transfer Center Nurse Specialist to join our team. This is a part-time, exempt position based at the Reverend Hugh Cooper Administrative Center. The work shift is varied days and hours.

Requirements

• 1 year RN experience or a minimum 1 year experience in a transfer center setting.
• Associates degree in Nursing required.
• State of New Mexico or Compact State Nursing License.

Nice To Haves

• Degree from accredited academic nursing program preferred.

Responsibilities

• Coordinate/inpatient bed assignments for critical care patients at Presbyterian.
• Coordinate, prioritize, and optimize patient flow activities throughout the hospital network.
• Coordinate and facilitate patient entry into Presbyterian from other health care facilities.
• Coordinate and facilitate the transfer of patients from other health care facilities to Presbyterian Hospital using Transfer center process flows.
• Collect clinical information to determine patient's level of care.
• Collaborate with Presbyterian on-call physicians to arrange acceptance by the attending physician specialist (uses intranet paging system).
• Communicate with referring hospital to coordinate transportation arrangements and provide an assigned bed.
• Ensure safe and timely placement of patients based on admission status, physician requested level of care, patient's clinical information and nursing units admission discharge criteria.
• Collect clinical information from patients, patients families, nurses, physicians and/or physicians offices to determine level of care for patient placement into an inpatient bed.
• Responsible for accurate and timely completion of patient registration and account data into the Hospital Computer System.
• Maintain 90% (or better) compliance accuracy rate with critical elements in account Registrations.
• Verification of demographic, guarantor and Insurance information.
• Use the account documentation process in the hospital's registration system to document missing information.
• Comply with all Revenue Cycle Education Policies.
