Transfer Center Coordinator (Per Diem, Variable)
About The Position
At NorthBay Health, the Transfer Center Coordinator is a critical role in the NorthBay Health system, facilitating both inbound and outbound transfers. The Transfer Center Coordinator is responsible for facilitating and coordinating patient transfers, both inbound/outbound, tertiary transfers (emergent and urgent), and consults from NorthBay Health to tertiary acute care hospitals. This role sets up systems to enable both cost containment and quality patient care. This position requires excellent communication and organizational skills to ensure seamless coordination between various departments, healthcare facilities, and external agencies. The Transfer Center Coordinator works collaboratively with all areas of NorthBay Health to facilitate timely and appropriate transfer of patients regardless of complaint or need. This position will require liaison with the case management/utilization review team as well as the Administrative Coordinator. Areas of focus are capitated patients, repatriations, emergent and urgent transfers, notification calls to health plans, and re-triage of pediatric/trauma/stroke/STEMI patients. The Transfer Center Coordinator acts as a liaison between the medical staff, nursing staff, pre-hospital personnel, referring centers, and external EMS agencies.

Responsibilities
- Facilitating and coordinating patient transfers both inbound/outbound.
- Coordinating tertiary transfers both emergent and urgent.
- Coordinating consults from NorthBay Health to tertiary acute care hospitals.
- Setting up systems to enable both cost containment and quality patient care.
- Liaising with the case management/utilization review team and the Administrative Coordinator.
- Managing transfers for capitated patients, repatriations, emergent and urgent transfers.
- Making notification calls to health plans.
- Re-triaging pediatric/trauma/stroke/STEMI patients.
- Acting as a liaison between the medical staff, nursing staff, pre-hospital personnel, referring centers, and external EMS agencies.
