About The Position

Angels of Care Pediatric Home Health is seeking experienced Registered Nurses (RN) and Licensed Practical Nurses (LPN) to provide direct, one-on-one medical care in the homes of infants, children, and adolescents. The role involves focusing on the medical needs and treatment according to the active plan of care, conducting ongoing patient assessments, administering prescribed medications, treatments, and therapies, coordinating care, educating family members, and implementing preventative initiatives to protect the quality of patient care. Angels of Care has been serving the medically fragile community since 2000 and emphasizes values of Heart, Advocacy, Love, Outreach, and Speed.

Requirements

• Active RN or LPN/LVN license
• Trach & Vent experience
• Ability to provide care in a client home setting
• Ability to make a positive and lasting impression

Responsibilities

• Provide direct, one-on-one medical care to infants, children, and/or adolescents in their homes.
• Focus on the medical needs and treatment according to the active plan of care.
• Conduct on-going patient care and assessments.
• Administer prescribed medication, treatments, and therapies.
• Coordinate care.
• Educate family members on patient clinical care to enhance positive outcomes.
• Implement preventative initiatives to protect the quality of care for patients.
